How to apply to become a resident
Our application process is designed to bring clients into a circle of care that honors their individual needs. Please note that clients must meet the following criteria:
Have an AIDS diagnosis or disabling HIV
Have an annual income less than $93,900
Require 24-hour nursing or hospice care
Be a San Francisco resident
Accept our admissions agreement
Be 18 years old or older.
Providers can make referrals by downloading the application below and filling out the form.
Accessing BRANCH
At this time only clients exiting our Residential Care Programs can become BRANCH Aftercare clients.

How to apply to become a MAC resident
Our application process ensures you receive the high-quality compassionate care necessary for a safe and supported recovery. To qualify for our specialized restorative care program, applicants must meet the following criteria:
Be 18 years of age or older.
Be approved for a gender-affirming surgery taking place in or near San Francisco.
Review and accept our admissions agreement
